Medicare: Enter either an 8-digit (MM | DD | CCYY) or 6-digit (MM | DD | YY) date of current illness, injury, or pregnancy. For chiropractic services, enter an 8-digit (MM | DD | CCYY) or 6-digit (MM | DD | YY) date of the initiation of the course of treatment and enter an 8-digit (MM | DD | CCYY) or 6-digit (MM | DD | YY) date in item 19.
Additional information for form version 02/12: Although this version of the form includes space for a qualifier, Medicare does not use this information; do not enter a qualifier in item 14.

The Medicare/Medicaid & Disability/Hospitalization tabs have been combined into one tab labeled Dates / Specialty / CMS. In the lower rht corner, in the CMS 1500 defaults area, there are dropdown boxes to allow you to override boxes 14 & 15 on the new 02/12 CMS 1500 form (paper & PrtCap).
On the payer 'Printed Claims Options' tab, there have been four chk boxes added so you can choose whether the entire field, or just a date (without the qualifier) will appear. (This allows you, for example, to suppress qualifiers for all your Medicare patients by updating the appropriate fields added to the Printed Claim Options tab).
On the Dates / Specialty / CMS tab, choose the date type to enter, make sure that correct date box is populated, Qualifier will automatically be entered.

Note - Medicare: Needs 'Cond | Illness/injury occurred'; OR 'Condition | First Consulted' on the Printed Claims Options tab, chk boxes... 'No Qualifier in CMS-1500 2/12 box 14' and 'Leave CMS-1500 2/12 box 15 blank'.
Note - Clearing house may tell them dates in box 14 & 15 have to be same; then try selecting Initial Treatment Date for box 14 (added 8/14/14)(Uses Condition | First Consulted with no qualifier).
Providers and suppliers have the option of entering either a 6 or 8-digit date in items 11b, 14, 16, 18, 19, or 24a. However, if a provider of service or supplier chooses to enter 8-digit dates for items 11b, 14, 16, 18, 19, or 24a, he or she must enter 8-digit dates for all these fields. [Uncheck both boxes on the printed claims options tab for 6-digit dates.]
